OUTPATIENT PRIMARY CARE SERVICES
The APT Foundation has treatment providers uniquely qualified to treat substance use in the context of holistic Primary Care.
Primary care medicine is medical care provided by a doctor who sees you regularly for wellness check-ups as well as minor medical problems, such as colds and flu. Primary care doctors may also help you manage chronic conditions like HIV, HCV, high blood pressure, diabetes and asthma.
Other areas of service include laboratory procedures, nutrition and exercise education, assistance with smoking cessation, immunizations, evaluation and treatment of acute concerns plus screening and counseling based on various categories of risk.

PRIMARY CARE STAFF
Our practitioners are affiliated with Yale New Haven Hospital and the Hospital of St. Raphael.
Our staff will interview you about any current symptoms you may be experiencing, ask you about your medical history and follow-up with a physical examination., Our primary care doctors are trained and have experience interpreting basic medical tests, such as blood work, urinalysis, electrocardiograms and x-rays, but they may refer you to specialists for more complex diagnostic procedures.
The APT Foundation is equipped to provide telehealth appointments in response to the latest guidance from the Connecticut Department of Mental Health and Addiction Services and state and local public health officials.
